Tammy R. Johnson age 61 of Ft. Wayne, IN passed away from this life on Friday, March 20, 2026 at University Park Rehabilitation and Healthcare. Tammy was born May 3, 1964 in Garrett, IN to Elbert Hugh and Dorthea A. (Berkas) Chittenden.
Tammy is survived by her son: Blake Johnson, Ashley, IN; 4 grandchildren; brother: Michael Chittenden, Garrett, IN; sisters: Sandra Stevens, Garrett,, IN and Kimberly Kessler, Garrett, IN.
Tammy was preceded in death by her parents: Elbert Hugh and Dorthea Chittenden; brother: Kerry Hugh Chittenden.
Visitation will take place from 11 a.m. – 1 p.m. Tuesday, March 24, 2026 at Thomas Funeral Home, 1277 C.R 56, Garrett, IN
Services will take place at 1 p.m. Tuesday at Thomas Funeral Home
Burial will follow in Christian Union Cemetery – Garrett, IN
Memorials are to the Phoenix Society for burn survivors.
